EMPLOYMENT

Please read and agree to the following statements:

I certify that all facts contained in this application to the best of my knowledge are true, complete, and accurate. I understand that any untruthes shall be gounds for dismissal from my employment with the City of Red Bay. I fully authorize an investigation of all statements contained within this application and attached resume and to give you all information concerning my previous employment and any pertinnet information they may have, personal or otherwise, and release the company from all liability or any damage that may result from the use of any and all such information. 

I understand and agree that no representative of the company has any authority to enter into any agreement for employment for any certain time period, or to make an agreement in contradiction to the foregoing, unless it is placed in writting, and signed by an authorized company representative. 

This waiver does not allow the release or use of disability-related or medical information in any manner that is prohibitted by the Americans with Disabilities Act (ADA) and any and all other relevant Federal and State laws. 

I also understand that any and all consumer credit report or criminal records checked might be necessary prior to my employment. If any such reports are required, I fully understand that, in compliance with Federal law, the company will provide me with a writted notice reguarding the use of these reports and will also obtain a seperate written authoritzation from me to consent to these reports. 

I also understand that a poor credit history or a conviction of a crime will not automatically result in my disqualification from employment. Finally, in compliance with Federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification documents at the time of being hired.
